Actually the easiest way to do what I think you are asking for is to use "gparted" to format the usb stick, and then use "DiskPup" to install Puppy.
"DiskPup" includes a dialog to select/create the directory that will contain Puppy.

You can add the parameter TZ to your kernel boot parameters, for example: ... pfix=ram TZ=EST5EDT ... but the TZ variable usually is the name of a timezone file, and the initrd does not have those files. If I may, I would like to make a suggestion concerning f2stickpup and multiboot support. What if by default, or as an option, the 'puppy' files were installed 2 dirs deep? Without much modification, I'm sure then the 'boot' facility could easily detect whatever pups are installed and modify the app...
